Our Mission

At Professional Home Health Care 2, we are dedicated to providing excellent care services to the Central Ohio community. Our goal is to assist in caring for each individual in a compassionate manner. We help provide a smooth transition from the hospital or other facilities to caring for our patients in their homes. We will care for each individual to meet their health care needs through an interdisciplinary team of physicians, nurses, STNA’s, personal care attendants, and therapists as needed. The health care team will work with each individual through the implementation of the physician’s plan of care. We will assist in empowering the patient and family members to provide daily care needs as much as possible and to enable them to reach independent living.
